“Cortisol belly” is everywhere at the moment. Saliva test kits, cortisol cocktails, supplements promising to switch the stress hormone off.

Almost all of it misunderstands what’s actually happening — and the reason is genuinely interesting.

Your blood cortisol is probably normal. The problem is that the fat around your organs manufactures its own supply, and no blood test will show you that.

Here’s what the research actually says, and what it means if you’re a bloke in his forties who can’t shift a gut.

The Paradox That Confused Researchers for Decades

Start with an observation that took twenty years to explain.

There’s a condition called Cushing’s syndrome, caused by genuinely excessive cortisol. It produces a very specific picture: fat around the middle, high blood pressure, raised blood lipids, insulin resistance, type 2 diabetes.

Now look at ordinary visceral obesity with metabolic syndrome. The resemblance is striking — researchers have described the similarities in exactly those terms.

Same fat distribution. Same blood pressure. Same lipids. Same insulin problem.

So the obvious hypothesis was that men with big waists must have high cortisol.

Except they don’t

Measure plasma cortisol in men with simple obesity and it comes back normal. Repeatedly. Across studies.

That’s the paradox. A condition that looks exactly like cortisol excess, in men whose cortisol tests are unremarkable.

For a long time nobody could reconcile it. Then the answer turned up in the fat tissue itself.

Your Belly Fat Makes Its Own Cortisol

This is the mechanism almost nothing written for a general audience explains, and once you know it the rest of the topic makes sense.

Your body carries a large pool of cortisone — an inactive form of cortisol. It circulates harmlessly, doing nothing.

Fat tissue contains an enzyme called 11β-HSD1. Its job is to convert inactive cortisone back into active cortisol.

So fat tissue doesn’t just respond to cortisol. It regenerates it, locally, inside the tissue.

Why that changes everything

Two findings make this more than a curiosity.

11β-HSD1 is elevated in the fat tissue of people with obesity. Not slightly — measurably, in both visceral and subcutaneous fat, with expression generally higher in visceral fat.

And it’s causal, not just correlated. Mice engineered to overexpress this enzyme specifically in fat tissue develop what researchers describe as a faithful replication of the metabolic syndrome — while their circulating glucocorticoid levels remain unaltered.

Read that last clause again. Blood levels normal, metabolic disease anyway.

The active cortisol is being made where it does the damage, and it never shows up meaningfully in a peripheral blood sample.

The plain-English version

Think of it as a thermostat problem, not a boiler problem.

Your adrenal glands — the boiler — are working fine. But the room where your organs live has its own heater, and that heater has been turned up.

Which is why “reduce your stress to lower your cortisol” is only half the story, and why the half everyone repeats is the less important one.

Why Your Cortisol Test Is Probably Useless

Now the practical implication, and the reason the current wellness trend annoys me.

The saliva and urine kits sold on Instagram measure circulating cortisol. We’ve just established that circulating cortisol is typically normal in men with exactly the problem those kits claim to detect.

You can have completely unremarkable blood or saliva cortisol and still have elevated glucocorticoid activity inside your abdominal fat.

So a normal result reassures you when it shouldn’t, and an abnormal result mostly reflects when you took the sample. Cortisol follows a steep daily rhythm — high on waking, falling through the day. Sample at the wrong hour and the number means nothing.

There is one situation where cortisol testing genuinely matters, and it’s a proper medical one. More on that below.

What actually tells you more: your waist measurement, your fasting insulin and your HbA1c. All three are cheap, all three are meaningful, and none of them depends on catching a hormone at the right moment.

The Insulin Connection

Here’s where the loop tightens, and where this article overlaps with everything else about a stubborn gut.

Research in lean and obese men found that insulin upregulates cortisol regeneration in adipose tissue. During an insulin clamp, cortisol-regenerating activity across fat tissue increased.

More striking: that regenerating activity was detected in the obese participants only.

So the sequence runs like this. Visceral fat raises insulin. Insulin drives the enzyme that regenerates cortisol inside fat tissue. Local cortisol promotes further visceral fat storage.

Round it goes, and the entry point isn’t stress at all — it’s insulin.

That’s why men who fix their sleep and meditate diligently but keep eating the way they always did are often disappointed. They’ve addressed one input to a loop with two.

What Cortisol Actually Does to a Man’s Body

Most content on this topic is written for a general or female audience. A few effects matter specifically if you’re male.

It suppresses testosterone. Cortisol and testosterone have an inverse relationship, and chronic elevation of one blunts the other. It breaks down muscle. Cortisol is catabolic. Less muscle means worse insulin sensitivity, which feeds the mechanism above.

It drives appetite — for specific things. Not for salad. Stress reliably pushes intake toward high-sugar, high-fat food, and this behavioural pathway may matter as much as the hormonal one.

It wrecks sleep, and poor sleep raises it further. Another loop, and one that’s very reachable.

Your visceral fat isn’t passive storage. It’s an active endocrine organ running several processes against you at once.

And Blocking It Didn’t Work Either

Before you go looking for something to switch the enzyme off — that’s been tried, properly, with real money behind it.

11β-HSD1 looked like an obvious drug target. If local cortisol regeneration drives metabolic disease, inhibit the enzyme and the disease should improve.

Pharmaceutical companies developed inhibitors and ran clinical trials.

The results were disappointing. One phase II trial did reduce HbA1c in a diabetic population. But trials in metabolic syndrome and obesity did not meet their primary endpoints.

What to take from that

The mechanism is real and well documented. The idea that you can bypass the work by blocking one enzyme is not.

Sleep, before anything else

One night of short sleep measurably raises next-day cortisol, and chronic restriction keeps it elevated.

This is also the intervention most British men are quietly failing. Fixed wake time seven days a week, dark cool bedroom, no alcohol within three hours of bed.

Get the alcohol down

Alcohol raises cortisol directly, disrupts the second half of your night, and adds calories you won’t account for.

Train, but stop overtraining

This one catches out the men who are trying hardest.

Hard training raises cortisol acutely, which is fine and adaptive. But six sessions a week on five hours’ sleep and a large deficit produces chronic elevation, and that’s counterproductive.

Three to four quality resistance sessions beats six mediocre ones. Rest days aren’t optional at 45.

Control the insulin side

Protein and fibre at every meal, refined carbs kept out of the evening, and a real gap between eating occasions rather than grazing from breakfast to bedtime.

Deal with the actual stressor

Breathwork and meditation genuinely help, and the evidence for slow controlled breathing shifting autonomic balance is reasonable.

But if the source is a job, a relationship or money, a ten-minute app session is managing the symptom. Not everything is fixable, and some things are — it’s worth being honest with yourself about which is which.

Where Fat Burners Fit — and One Caveat Specific to This

Worth addressing, because it’s the obvious next question and there’s a wrinkle most people miss.

No fat burner touches cortisol. Not one of them affects the enzyme regenerating it inside your fat tissue, and none is sold on that basis.

When to Actually See Your GP

Almost everything above is about ordinary stress and ordinary visceral fat. There is a genuine medical condition worth knowing about.

Cushing’s syndrome is rare but real, and it’s the one situation where cortisol testing is exactly the right thing to do.

Features that would prompt a GP to investigate include rapid central weight gain with thin arms and legs, a rounded and reddened face, purple stretch marks wider than about a centimetre, easy bruising, muscle weakness particularly in the thighs, and new or hard-to-control high blood pressure or diabetes.

If several of those apply, especially the stretch marks and the proximal weakness, that’s a GP appointment rather than a supplement.

Also worth flagging: long-term steroid medication — for asthma, arthritis or inflammatory conditions — can produce the same picture. Don’t stop prescribed steroids, but do mention the weight change.
